New Patient Registration

We are currently registering patients with Dr Amy Howarth as their Named GP.

If you live within our practice area, and are eligible to receive NHS treatment, you can register at the practice.

Our Registration Pack can be downloaded using the links below or collected from Reception. Please bring the completed Forms to Reception together with the required ID.   Please ensure you complete all of the GMS1 form, including your NHS number, as missing information prevents your notes from being transferred to us electronically.  You can obtain your NHS number from your previous practice.

If you are unable to attend during the working day, you can call in at Glenlyn Medical Centre on most Saturday mornings and some evenings - please call to confirm we are open before visiting.

NHS patients who are be visiting the area for a short period of time can register as a Temporary Resident for up to 3 months. Please ask at Reception for a Temporary Registration Form.

The practice can offer a private, fee paying, service to persons not entitled to NHS treatment. A fee list is available at Reception. 

If registering a new born, please bring a copy of the Full Birth Certificate and Red book. This is needed before booking baby's first 8 week check and immunisations.
